The Senate recommends that:

1. the attached forms (numbered 1A, 1B, and 2) (Available by Request from the Senate Office) be the standard forms used to report the results of peer of colleague evaluation of teaching by individual faculty members as specified in Section II.B.2 of Senate Proposal 2-87.
   
   
2. the responsibility for monitoring and amending this policy will rest with the MTU Faculty Senate. The Senate Instructional Policy Committee will be responsible for recommending changes in these standard peer/colleague evaluation report forms to the Senate.
