Staff Hiring Toolkit

Employment Services developed this page as a resource to departments as they welcome new employees to our University.

We all want a new hires first day to be a warm and welcoming experience as they learn more about the University and meet their new colleagues. However, some legal paperwork (for example, Form I-9, Section 1) is required to be completed by their first day of work for pay.

Many of the forms below can be completed at any time after the candidate's acceptance of our official job offer letter and before their first day. We allow new hires to print the forms below, complete, and bring along on their first day; however, HR Employment Services has employment packets ready to go for new hires with all necessary forms.

New Hire Paperwork

Federal laws require HR Employment Services to verify a new hires identity and employment eligibility, but Payroll Services also needs documents to ensure that new hires are compensated properly and in a timely manner.

For Form I-9, the employer must complete Section II within 3 business days of the first day of work for pay. This timeframe is critical, but even more dire, if HR must verify the employee through the Department of Homeland Security's E-Verify Computer System.

Required Legal Paperwork & Documentation
HR Employment Services must "attest, under penalty of perjury, that we have examined the document(s) presented by the new hire, that the document(s) appear to be genuine and to relate to the new hire, that the employee began employment on (month/day/year) and that to the best of our knowledge the employee is authorized to work in the United States."

All new hires will need to provide us with the following:
Federal & State Form Requirements:
1. Form I-9, Employment Eligibility Verification * NEW VERSION
    The document selection from List A or List B and C are exclusively the new hires choice (see pg.5).
    Employers CANNOT specify which document(s) they will accept from an employee for this form.
    All unexpired documents must be presented in-person to HR.
2. Federal W-4 (2012) * NEW VERSION
    The new hire must present their Social Security Card in-person to HR.
3. Michigan W-4 (2012) * NEW VERSION
    The new hire must present their Social Security Card in-person to HR.
Michigan Tech Form Requirements:
1. New Employee Information Expectations
    The new hire must indicate compliance with all University policies and regulations by reviewing and signing this form.
2. Employee Patent, Research, and Proprietary Rights Agreement (includes grad students) * NEW VERSION
    The new hire must indicate the reading of both sides and the understanding of this agreement by signing this form.
3. Board of Control Policy re: At Will vs. Satisfaction Employment Relationship (non-union staff only)
    The new hire must indicate the selected employment status by signing the Acknowledgment Form on the last page.

4. Beneficiary Designation Form
    The new hire may designate payment of all earned compensation at death by completing and signing this form.
Michigan Tech Forms - Optional:
1. Emergency Notification Form
    The new hire may designate who should be contacted in the event of an emergency by completing and signing this form.
2. Payroll Direct Deposit Authorization Form
    The new hire may authorize the deposit of the payroll check to a financail institution by completing and signing this form.
3. Post Hire Equal Employment Opportunity Form
    The new hire may provide data used to fulfill reportings requirements by completing this form.

What are the next steps for a new hire?

Encourage new hires to contact Benefit Services at 906-487-2517 to schedule a time for their Benefits Orientation. Since HR is located in the Lakeshore Center, many new hires find it convenient to make one visit for both completion of New Hire Paperwork (with Employment Services) and the Benefits Orientation (with Benefit Services). Both offices are located on the 2nd floor of the Lakeshore Center.
