Change-of-Status
The following process is used when making a change to a current faculty contract or when extending an Academic Faculty Appointment to a current MTU staff member. Situations are given for:
- Academic Faculty
- Research Faculty
For each given situation, the materials listed need to be forwarded through the Dean's Office to the Provost's Office.
Academic Faculty
Tenure Track (Professors: Full, Associate, Assistant)
- Leaves of Absence (for all leaves except sabbatical)
- Employee Status Change Form (Gold)
- Faculty request for leave (letter or memo)
- Administrative Appointments
- Other
- Add Adjunct Appointment
- Rank Change with no PhD Completion
- Employee Status Change Form (Gold)
- Rank Change with PhD Completion
- Employee Status Change Form (Gold)
- Discipline/administration title (uncompensated)
- Employee Status Change Form (Gold)
- Return from Leave of Absence
- Employee Status Change Form (Gold)
Note: Send directly to Human Resources
Non-Tenure Track (Lecturer Ranks, Professor of Practice, Instructor, Adjunct, Visiting, ROTC Faculty, Emeritus)
- Faculty
- Change in current contract
- Employee Status Change Form (Gold)
Note: If you are extending the end date on an existing contract, it must be processed as a reappointment. Complete a Form A for temporary instructors, adjuncts, visiting. Complete a Form L for lecturers.
- Compensating Adjunct Faculty
- Staff
- Academic Appointment or Reappointment
Research Faculty (Professor, Associate, Assistant)
- Change in current contract
NOTE: This may include a concurrent appointment, such as an adjunct appointment in the same department or another department while having a research faculty appointment.

Note: If you are extending the end date on an existing contract, it must be processed as a reappointment.
