Subject: New requirements for new international students and scholars for
 social security number application

Attention MTU Departments/Units:

There are specific requirements that must be specified in the department
letter that new international students and scholars must present to the
local social security office to verify employment at MTU  to apply for a
social security number.  The requirements are as follows:

1)  must be present in the U.S. for 10 days or more

2)  letter must be typed on department letterhead

3)  name of international student or scholar should be typed

4)  type of employment-job description/nature of job-will receive wages
for employment

5)  start date

6)  number of hours of work per week

7)  MTU Employer Identification Number-38-6005955

8)  Department Telephone Number

9)  Immediate Supervisor original signature

10)  Name and title of Immediate Supervisor

11)  Date
