25.1.2. Certificates of Insurance

Definition

Certificate of Insurance: A document proving the University has insurance coverage.


General Information

A proof of insurance certificate may be needed by employees or departments when contracting with outside venders or when signing a contract for research projects. Certificates of insurance are available for automobile, general liability, property, and errors and omissions.


Procedure

To obtain proof of insurance, do the following:

  1. Send a written request and/or a copy of a contract, if applicable, to the office of Risk Management, room 508 of the Administration and Student Services Building, with the following information:


    1. To whom the certificate is to be issued


    2. Name of group or individual requesting the certificate


    3. Individual responsible during the activity


    4. Events or services: purpose, location, time period


    5. Type of insurance and limits for which proof is required
