18.3.4. Reporting Incidents and Injuries
Definition
Incident: Any unintended event that causes injury or property damage or has the potential for causing serious injury or property damage.
Policy
Students and employees must report all incidents to their supervisor immediately. Supervisors must investigate incidents to identify the cause(s) so that corrective action can be taken. The investigation results must be reported to Occupational Safety and Health Services (OSHS), within 24 hours, on the form provided for this purpose.
Procedure for Injuries
The following instructions should be followed when responding to injuries:
- Request an ambulance for serious injuries requiring emergency assistance by following the procedure for emergency reporting.
- For minor injuries, request first-aid assistance from Public Safety if needed, by dialing 7-2216.
- Injured employees requiring non-emergency medical treatment should be directed to the University Health Center or, after hours and on weekends, to the Portage Health System walk-in clinic located at the hospital emergency room.
- Transport an injured person to a medical facility using private transportation only if doing so will not risk further injury.
- All student and employee injuries must be reported to the immediate supervisor or instructor.
- The department must complete an investigation report, available from OSHS by calling 7-2118, and forward it to OSHS.
Procedure for Reporting Injuries and Incidents
To report an injury and/or incident, do the following:
- Obtain an Incident and Injury Investigation Report from the departmental office or by calling OSHS at 7-2118. Refer to the OSHS Web site for more information and a copy of the form.
- Conduct an incident investigation using the report form as a guide.
- Send a copy of the report form to OSHS within 24 hours or as soon as possible.
