17.2. Fund Authorization
Policy
Once a fund has been established at the Michigan Tech Fund for any department or office of the University, appropriate authorization must be given of who can authorize documents and/or access information and check requests or transfer on the funds. A Fund Authorization form must be on file at the Michigan Tech Fund.
Procedure
To establish fund authorization do the following:
- Complete a Fund Authorization Form, available by calling the Michigan Tech Fund finance department at 7-1930, or from the Web site <http://www.mtf.mtu.edu/resources/pages/forms.html>, with the following information:
- Fund Number
- Fund Name
- Date
- Department
- Purpose of Fund
- If completing the form to indicate the fund manager, complete the Fund Manager box on the form with the following information:
- Full name of fund manager
- Signature of fund manager
- Social Security Number of fund manager
- Limitations, restrictions (if any)
- If completing the form to indicate a designee (a person other than the fund manager), complete the Designee box on the form with the following information:
- Full name of designee
- Signature of designee
- Social Security Number of designee
- Limitations, restrictions (if any)
- Designated Michigan Tech Fund Employee for Interdepartmental Charges (required to complete) box on the form with the following information:
- Full name of designee
- Signature of designee
- Social Security Number of designee
- Limitations, restrictions (if any)
- Have the appropriate dean, department chair/director or immediate supervisor sign the form.
NOTE: Complete one form for each fund.
- Return completed form to the Michigan Tech Fund finance department.
