10.7.1.2. Telephone Equipment


Policy

To ensure the reliability and maintainability of the campus phone system, all telephone equipment must be purchased or leased through Telcom. This includes, but is not limited to: single line phone sets, key system (Panther) sets, and cordless sets.


Procedure

To request phone service, do the following:

  1. Complete a Service Request form, available from the Web site <http://www.tc.mtu.edu/cs/forms/phonereq.html> or send an e-mail request to <telcom_request@mtu.edu> with the following information:


    1. Requester's name, phone number, and e-mail address


    2. Contact name, phone number, and department (if different than requester)


    3. Valid University index number to be charged for the installation


    4. Valid University index number to be charged for the monthly fees


    5. Service recipient


    6. Location(s): Building name and room number


    7. Jack ID (if available) or location of jack


    8. For buildings with new jacks (Dow), indicate by jack color

      For buildings with old jacks, indicate by location

    9. When someone will be available at location


    10. Where can key be picked up if needed


    11. Cable needed (type and length)


    12. Type of phone


    13. Phone number serviced


    Requests are scheduled within 6 to 8 working days upon the confirmation date, depending on request.
