10.3.6. SURVEY


Definition

SURVEY: A software program that captures bubble sheet information and creates reports.


General Information

The reports generated from the SURVEY program can have weighted response sets where each response can have a separate value. The program can capture and process up to 10 responses for each question. The basic 40 question sheet is designed to allow the questions to be printed directly onto the bubble sheets. Projects that can be done by SURVEY are: balloting, supervisor evaluations, summer youth program evaluations, teacher evaluations, wellness and about 30 other special programs. The software is flexible and allows for varied output formats. The software also allows for outputting the raw data in several formats for use in other statistics packages.


Procedure

To have a report or bubble sheet(s) read, do the following:

  1. Bring a draft copy of the survey to Data Solutions in room 217 of the Administration and Student Services Building.


  2. Consult with DS staff to have survey created.


  3. NOTE: There is a charge for the forms.
