2011 Michigan Tax Return Preparation Guidance

NOTE:  This is just a summary of common line items.  It’s important to thoroughly go through every line item and do applicable calculations.

    Instructions

Line 4Enter School District Code of your U.S. address.  Complete List of School District Codes are found on pages 49-50 in the instructions. (This is actually pages 27-28 of the on-line instruction booklet, as the on-line instruction booklet does not contain forms).  Most common School District Codes are:

  • Houghton:  31110
  • Hancock:  31010
  • Dollar Bay:  31100
  • Lake Linden:  31130
  • Calumet:  31030
  • Chassell:  31050

Line 6:  Leave blank.

Line 7:  Check “Single” box.  Nonresident individuals file as single.

Line 8:  Check “Resident” box.

Line 9a:  Number of exemptions should be “1” and $3,700 should go next to it.

Line 9h: Amount should be $3,700.

Line 10:  Enter the amount from Line 10 of your Form 1040NR-EZ or Line 36 from 1040NR.  This is your adjusted gross income.

Line 11 & 13:  Complete Michigan Schedule 1, if applicable.  Otherwise – put $0. See instructions pages 12-14.

Line 18:  Complete Michigan Schedule 2, if applicable.  Otherwise – leave blank. See instructions pages 15-16.

Line 31:  Enter amount from Box 17 of W2 Tax Form.

Final Notes:

  • Be sure to sign and date return.
  • Bottom of Form indicates where to mail form. 
  • Attach State Copy of 1042-S Tax Form to Tax Return.
  • Make copy of return for your records.
  • Be sure to complete Michigan Schedule W.
